To begin accessing our services, please fill out our Initial Intake Form, and the appropriate service agreement: our Service Agreement with In-Office Visit, if you want LOMAP to travel to your office for a consultation; or, our Service Agreement without In-Office Visit, for all other LOMAP services.
These are in form format, so you can simply enter your information, save the documents, and email them as attachments to Rachel.
